Your puppy was born from Mini Puppies Trudi (registered with AKC) or Weavers Little Jasmine and Weaver’s Theodore (born from Neisha and Chaos, both AKC registered). The puppy from this litter is being purchased without AKC registration papers.
Your puppy was born on September 5, 2010.
Your puppy received his/her first shot on October 22, 2010. He/she received a vaccine called Intervet Proguard 7. He/she also received Neemex wormer on October 8 and 22, 2010.
On the date of purchase as signed below this puppy is found to be healthy. Your puppy is guaranteed for one year from the date of purchase two years with the purchase of Nu Vet Vitamins. This guarantee covers any unforeseen life threatening illness which cannot be treated and cured. We do not cover any veterinary expenses once the puppy leaves our possession on the date of purchase for any reason. Guarantee is for replacement of puppy. No cash refunds will be given under any circumstances.
Please take your new puppy to your veterinarian by October 30, 2010.
