PUPPY/DOG PURCHASE AGREEMENT
This BILL OF SALE is to certify that on the ________ day of __________________ 20______
Ruth Kurilich, hereinafter referred to as the Seller, has sold the dog
___________________________________________, AKC registration ______________________________,
To ________________________________________,
Hereinafter referred to as the Buyer for consideration of the purchase price of $________.
The Buyer and Seller mutually agree as follows:
1. The Seller guarantees that she has full power to sell the dog, further more the title is free and clear from any liens.
2. Buyer agrees to purchase the dog as is. The Title of the dog will be transferred when the full purchase amount has been paid.
3. Buyer agrees that upon receiving of the dog __________________on the _____ day of __________ 20____
that all responsibility of the dog shall transfer to them from that day thereafter.
4. Buyer agrees to the health guarantees of the puppy/dog as follows;
a. The health of this dog is guaranteed for 10 working days through examination by a licensed veterinarian
at the option and expense of the Buyer. If the dog is found to be in unsatisfactory health and written documentation of the condition is presented to the seller in those same 10 working days, the Seller shall
refund 100% of the amount paid on the purchase price provided dog and any registering papers are returned
to the Seller.
b. 2year 3month hip dysplasias guarantee with full registration only and only with proof of hip dysplasia through x-ray by a licensed veterinarian at the option and expense of the Buyer. Documentation of the hip dysplasia must be presented to the Seller in the same 2year 3month period to be valid. Buyer understands and agrees that the dog will be x-rayed by the Sellers licensed veterinarian to verify the hip dysplasia if no hip dysplasia is found the dog will NOT be replaced. If the hip dysplasia is confirmed by the Sellers licensed veterinarian the Seller will replace the dog as soon as possible with a replacement of equal value to the dog at the time of the original purchase. The Buyer has the option to keep or return the original dog. There is no hip dysplasia guarantee if buyer gets no papers.
5. If checked Buyer agrees to spay/neuter the dog by 9months of age and to provide documentation to the Seller in those same 9months. Buyer will receive a $100 refund from the Seller only after proof of spay/neuter has been given to the Seller within those same 9months. If for any reason the Buyer does not comply Buyer agrees to relinquish the dog and that the Seller will collect the dog as soon as possible.
6. The Seller and Buyer certify by signing this bill of sale that this dog is not sold for resale purposes and that the Buyer will inform the Seller if it is necessary for this dog to change hands and the first right of refusal will be given to the Seller.
7. This BILL OF SALE represents the entire agreement between the parties. No other agreement or promises,
Verbal or implied, are included unless specifically stated in this written agreement.
8. This BILL OF SALE is made in the State of Ohio, and shall be enforced and interpreted under the laws of the
State of Ohio. Should any clause above be in conflict with State law, that individual clause shall be null and void. When both Buyer and Seller sign this contract, it will then be binding on both parties.
