ALTERATION AGREEMENT

I agree to have this dog altered (spayed or neutered) between the ages of 6 months and 10 months. The AKC registration form will be withheld until seller receives a statement verifying the alteration from the veterinarian doing the surgery. Failure to have the dog altered within the agreed time period voids all guarantees after one year of date of sale.

COMPANION DOG GUARANTEE

The minimum standards as accepted by the MPDBA are as follows:
"COMPANION DOG" means this animal is sold as a pet rather than for breeding or show purposes. No other warranties or guarantees, expressed or implied, are made under this contract except as stated below:

1. The dog is purebred and pedigree is correct and given at time of sale. Dog is registerable with the AKC, unless form is withheld per written agreement. REFUND: Cost of puppy minus for the deposit, provided living dog and registration papers are returned to seller.

2. This animal is guaranteed for 30 days at the option and expense of the buyer. If the dog is found to be unsatisfactory, the seller shall refund 100% of purchase price provided living dog and registration papers are returned to seller.

3. This animal has the following known health problems:
Breed tendencies: There is absolutely no guarantee on the coat whatsoever, not the color texture or length of the coat

4. This animal is guaranteed to be free of hereditary crippling/disabling defects apparent by 24 months of age. Determination of what constitutes a hereditary defect is to be by the unanimous agreement of two (2) veterinarians agreeable to both parties. One of these veterinarians is to be a tenured faculty member of an accredited university veterinary teaching hospital or Board Certified in the applicable specialty. Cost of the opinions is to be assumed by the buyers. Choice of remedy is at buyer's option: (Circle option choice and buyer and seller initial)

a. Refund of 100% provided the living dog and its registration papers are returned to seller.

b. Adjustment (to be filled in at time of sale): Seller to pay veterinary bills for treatment of
said defect. Total liability shall not exceed the purchase price of the animal.

5. This dog is guaranteed to be of good temperament at time of sale.

6. Buyer agrees to: Annual veterinary checkups, including heartworm check; to keep all immunizations current according to AVMA guidelines; to properly administer heartworm preventative; to house this dog in their home and, when outdoors; to confine him in a fenced yard or on a leash.

7. Beginning in the year 2009 and continuing until the dog reaches at least the age of 9 years, Buyer agrees to obtain annual eye examinations by a certified veterinary ophthalmologist and provide breeder with a copy of the results.

8. Buyer agrees to attend puppy socialization classes (if age allows) and/or dog obedience classes with this dog.

9. If buyer decides to sell or otherwise dispose of this dog, seller has the right of first refusal. If, as a result of sellers refusal, this dog is placed with a new owner, buyer agrees to furnish seller with the name, address and telephone number of new owner. Seller agrees to accept a permanent return of this dog if buyers are relinquishing for no monetary compensation.

It is understood that this deposit in the amount of (check # ) is a non-refundable deposit until delivery of said dog to buyers, at which time it will be applied to the purchase price and will be refunded only under the terms of this agreement. If Buyers do not take possession of said dog by ____________, this contract is null and void and this deposit will be forfeited.
