Have dog spayed or neutered prior to age 9 months. Not doing so voids this guarantee. I(We) agree to provide proof of spaying/neutering to Karlyn’s within one month of the
procedure in the form of a letter from my(our) veterinarian on his/her letterhead OR by mailing a copy of the bill for the procedure with the veterinarian’s name, address, and phone number clearly identified. I (We) understand that Karlyn’s will withhold AKC registration papers until proof of the spaying/neutering is received. I(We understand that Karlyn’s health guarantee is void if the spaying/neutering is not done BEFORE the dog is 9 months old. This
Lhasa Apso should be neutered/spayed on or before 00/00/200?.

Provide plenty of socialization and training classes to assure the dog will not become the dominant member of the household.

Keep the dog on leash when it is not in a fenced area.

Be responsible for all liability in case of accidents while the dog is in his/her care.

Contact Karlyn’s if there is ever a need to place the dog in another home. Karlyn’s will either take the dog back or help the buyer find a new home for the dog if necessary. Transportation fees are the responsibility of the owner. In the event that Karlyn’s is no longer available to take the dog, the Lhasa Apso may be turned over to the national Lhasa Aspo Rescue organization. If on his/her own the buyer has found a new home for the dog, he/she will notify Karlyn’s for approval of the new home. At no time will the buyer turn this dog over to any humane society, animal shelter, commercial breeders, or brokers.

Karlyn’s guarantees the puppy identified above to be free of hereditary crippling/disabling defects visible by 24 months of age that would effect it’s ability to be a quality companion dog. Determination of a hereditary defect is to be made by a licensed veterinarian. The puppy will be replaced with a puppy of equal value provide appropriate documentation, from a licensed veterinarian, describing the defect is provided. This guarantee does not cover veterinary expenses, and is not transferable. Karlyn’s will not replace a dog if we are not advised of him/her being ill, are not provided with veterinary documentation of the dogs condition, or of the necessity of him/her being euthanized beforehand.

Provide proper maintenance and emergency health care, including but not limited to, annual checkups, vaccinations, and heartworm tests. Buyers must keep all vaccinations current and administer heartworm preventative as prescribed by a veterinarian. Failure to do so voids
this guarantee. This guarantee does not cover injuries or illness caused by accidents, neglect, or abuse, including improper diet and improper feeding, and improper grooming and coat care, or inadequate housing.

For provisions of this guarantee to be in full effect, the dog identified above must be taken to a licensed veterinarian for an initial exam within 5 business days of purchase. If a health
