This is an application for a Boreayl Siberian Husky puppy. Please describe your family and the reason for desiring a Siberian Husky.
Please list past and present pets that you have owned including age and breed.
If you no longer have these pets, what became of them?
Do you currently own or plan to own any small animals (cats, birds, rabbits, hamsters, etc.)or livestock?
Is there any reason you would ever consider giving up a pet? If so, please explain.
Where do you plan to keep your Siberian Husky while you are at home or away?

Our contract requires all owners to have their puppies CERF tested so that we can maintain a comprehensive eye health history of our line. This typical cost is between $25 and $75. Are you willing to honor this requirement?
