Questions
What kinds of previous experiences have you (and/or the members of your household) had with dogs? What breeds?
Are you a breeder?
Yes      No
Do you have children? If yes, what ages?
Do you have other pets? What kind? What age?
How much time during the day/night will the dog be alone?
Where will your dog stay during this time?
Please describe how your dog will be contained and when(inside, outside, both, size of yard, fenced, etc.)
Do you plan to join any clubs or do basic obedience training with your dog?
Please describe why you want this toy breed dog? Breeding? Show? Pet? Therapy? Agility?
Would you accept Limited Registration on your dog? (Limited registration restricts entry in conformation shows or breeding. It does allow placements into obedience, lure coursing, agility, tracking, and earth dog events. Limited registration prevents you from having registerable puppies through the AKC should your dog become bred or breed another. )
Additional Comments or Questions
Are you able to drive or fly to our home in Northwest MO or meet in Kansas City MO to pick up a puppy?
Please provide us with a past vet and phone information for a reference. Also trainers, or pet sitters make good references too. The more the better.
What will you do if your dogs health requires a 1500 dollar surgery at a young age?
