Thank you for your interest in a dog from Kaos Siberians. We are serious when we say that our commmitment to this dog is for its lifetime, so this questionnaire is designed to make sure that our puppy is a good fit for your home or family.

* * Where do you live and how long have you been at your current address? Do you rent or own your home? If you rent, can you provide your landlord's contact information to confirm you have permission to have a dog at your residence?

* Are you married or single? Are all of the members of your household in favor of adding a Siberian Husky? Do you have children? If so, please state ages.

* Have you ever owned a Siberian Husky before? If not, have you researched the breed? Are you familiar with the socialization needs of a Siberian Husky and are sure you can meet these needs?

* Do you currently have other dogs or pets? If so, state species/breed and sex/age. If you have had pets in the past, please briefly state where they are now.

* If you don't currently have any dogs, have you ever owned a dog before? What happened to them?

* What are your reasons for wanting a Siberian Husky? (Show dog, pet, obedience, sled racing, agility....etc.) If you have a preference for sex/coat color or eye color, please let us know.

* We require all pet puppies to be spayed or neutered. Do you have any problems with signing a contract stating you agree to have your dog spayed or neutered? If yes, what is your reason?

* We, as breeders, strive to breed Siberian Huskies to be physically and mentally sound. This does not mean that all of our dogs are perfect. If a problem arises, we want to be contacted immediately with full details, whether the problem is genetic or otherwise. Will this be a problem? If yes, why?

* Are you interested in a puppy or would you consider adopting an adult dog?

* Kaos Siberian puppies receive only the vaccines required by state law. All of our health guarantees are void if they are vaccinated beyond what is agreed to in writing between the breeder and buyer. Will this be a problem? If yes, why?

* Do you believe and crate training a dog? If not, please explain why.

* Do you understand that regardless of training a Siberian Husky can never be trusted off leash? If you disagree, please explain why.

* Do you understand the prey drive of the Siberian Husky and that they can never be completely trustworthy with cats and other small animals, including small dogs? If you disagree, please provide your reasoning.

* Please add anything else about your lifestyle and family that will help me place the right puppy in your family.
