Whelped on August 18th 2008. First Worming at two weeks of age using Strongid T Wormer with weekly follow up worming.
First Parvo, Hepatitis, Leptospirosis, Distemper Shot (shots given 3-4 weeks apart in a series of 4 shots:

Feeding Instructions:
Dry Dog Food: Purina Puppy Chow (not the one with the maroon on the bottom of the bag). I prefer to free feed, which means they have kibble in front of them all the time.
Some dogs like to have their dog food softened and warm and this is how I do that:
Boil water, then remove from burner.
Add water to kibble until the kibble is covered with about 2 inches with water on top of it.
Cover and let sit until cool enough for them to eat safely. You can also add ice cubes after the food is soften.
Canned Dog food: Pedigree Puppy Lamb & Rice (1 can per feeding, twice daily.)
Sience Diet Land & Rice or A.D. Perscription
I prefer to feed puppies between 6 weeks to 9 weeks 3 x per day and from 10 to 12 weeks twice a day. After that they can have 1 can 1x a day with kibble free feed. Other Foods: Cottage Cheese, Plain Yogurt, Sour Cream, at cool to room temp. Cooked rice, boiled hamburger, chicken noodle soup at warm to room temp.
*These are particularly good for dogs with upset stomachs or dogs with diarrhea, or they just aren’t feeling well. Note: I prefer to substitute Cottage Cheese for canned dog food.
Water: Leave plenty of water available for puppy all the time. Water should be cool to room temp.
Toys and Treats:
Recommended: Bit a Bone Products, Kong Products, Soft squeeky toys (without glossy paint or things that rattle inside) Tennis ball toys, Sock toys (*but be carefull and watch that the dog does not get the garment frayed. It is good idea to stick with non-colored products like white for natural because some dyes are not good for animals and while they are fun to play with they are not good to leave with the puppy while unsupervised because they can chew them up and ingest them. Recommended treats: Milk bone dog biscuits, Liverwurst bits.
I do not recommend: Rawhide chews (many are treated with toxic chemicals like formaldehyde and like the rope toys can require life threatening intestinal surgery if a portion gets stuck in the intestine. Glossy latex squeaky toys painted with lead paint should not be given to your dog! Any toy that has small objects that rattle in it can be ingested or a toy that is too small that it can be swallowed by the puppy.

If during a period of 36 months from this date: _____________________________it expires or is diagnosed thru no fault of the owner with a hereditary, heart or cancer condition. Breeder agrees to replace the puppy free of charge providing a veterinarian exam is given and full oral disclosure and written documentation of the condition and procedure to treat are provided. The condition must be in-operable or or cost of surgery prohibitive of the cost of the dog and/or theprognoses of quality of life thereafter very poor. In the case of death, a death certificate is provided regarding the date, time and cause of death. This information must be furnished to the breeder within 30 days of the diagnosis or demise by a licensed Veterinarian and if need be the owner also authorizes the breeder and/or her agent to contact their said Veterinarian, allowing full release of any additional pertinent information. If this puppy was shipped to you the cost of shipping a replacement puppy will be the responsibility of the owner not the breeder.
In the event the puppy is sold or transfers from the possession on the original buyer herein known as the Owner, this agreement is null and void.
