Introduction
Thank you for your interest in Chekia Shelties. We do not have puppies very often, but we are admittedly particular about where they spend the rest of their lives. If you are not interested in owning a Sheltie in which the breeder maintains a lifelong interest, please do not fill out the application.

Have you had a Sheltie before - Have you had a dog before - When, and what became of them? (List any current dogs and other pets also)
What is your home situation - single adult living alone, two working parents with children (please list ages) - stay at home mom or dad - retired individual or couple - or other ? Please elaborate on schedule, how many hours daily the dog will be home alone, etc.
Do you live in a house, apartment, condo, mobile home (park?) ?? Size of yard, type of fencing and gates and latches? If you rent, do you have landlord permission to have a dog, and what would you do if the building was sold and the new landlord did not allow pets?
Are you familliar with, and comfortable with such facets of alternative health care as minimal vaccination, fresh food feeding, homeopathy, herbal medicine, chiropractic? Please elaborate
What are you particularly looking for in a Sheltie, and is the primary purpose you hope this puppy will come to fulfill, and what, if any, physical or behavioral characteristics would cause you be disappointed in this puppy at maturity? (Please include any requirements as to sex of puppy, colour and markings)
What would you do if your Sheltie became ill or injured and it was determined that the cost to restore his health would be ten thousand dollars?
Will your Sheltie be primarily a house dog with access to your entire home, an outside dog with occasional access to a room in your home, or other (please elaborate) ??
