Health Guarantee / Pet Contract

This puppy is being placed as a pet/companion animal and must be spayed/neutered by the age of 8 months old. Documentation (invoice) from your veterinarian must be sent to show the spay/neuter has been done by the age of 8 months old in order to receive the application for the limited (non-breeding) AKC registration papers.

Other than to neglect and accidents; to include proper nutrition and medical care; we guarantee this puppy for 5 years against Crippling Hip Dysplasia, Elbow Dysplasia, PRA, Hereditary Heart defects, PN and CHD (Dwarfism). If your puppy develops one of these disorders let me know immediately. At this point I will need verification from two veterinarians of the disorder. Hip certifications need to be verified by the OFA after 2 years old and before the age of 3 yrs. After verification of any disorder you will be provided with another puppy/dog (NO CASH refunds) of equal value. Your new puppy needs to be examined by your veterinarian within the first 3 days, any shipping and health care (vet) involved is at the new owner’s expense.

The new owners are responsible for providing their new puppy with a safe living environment, this includes a properly fenced in yard (chainlink, privacy fence, etc.) and/or kennel run. The puppy/dog is not to be chained out, or allowed to run at large. A shock collar is NEVER to be used on puppy/dog at any age for any reason. Your new puppy must go to a puppy socialization class and at least a beginner’s obedience class. This will help you and your new puppy adjust to life together. The new owner must stay in contact with the breeder and provide a yearly update with pictures. If at anytime the new owner cannot keep their new puppy, they must contact the breeder so placement can be made. The new owners are not to re-home puppy/dog without contacting the breeder first.

All puppies pet and/or show must have the prefix Ice Age’s on the puppy/dogs registration name and this cannot be changed for any reason.

The buyer agrees to adhere to the terms of this contract. If the buyer does not follow the terms set forth they forfeit all rights to the health guarantee and the AKC limited registration application form.
